Here goes:

You will have more of this than you will ever need, a couple guys could split a batch.

36 x 25 x.2 (for linkage)
36 x 25 x .5 (for linkage using Fastaway guard)
36 x 25 x 1 (for swingarm)

I would suggest getting a bag of each as its likely that fit can vary slightly bike to bike. You want the shim to prevent the ring from walking out but must not bind. The fit is close over the sleeves and may need a little dressing.

24 x 4 (linkage/swingarm)
15 x 4.5 (lower shock seal replacement)

If you have a Fastway guard, the included bearings are usually pressed in a little farther than the stock part. To allow the ring to ride in the proper position on the sleeve cap, use a 22 x1.5mm width oring under the 24 x 4. This is just for spacing purposes not for sealing. If you replace bearings in the Fastaway just press them in to stock dimensions.

By far the stickiest, HD grease I have tried. Do it right and forget about it for a long time. A year later it will look the same.

EDIT: I see those links do not point to the exact page I copied from, sorry. Shims are metric stainless steel. Orings are metric general purpose BunaN.

I found everything except the 15x4.5 o-ring. Should I try and get close with an inch sized o-ring instead of a metric?
Any help is appreciated.
